Please complete and return the following forms.

Your deposit is required with your application and no application will be accepted without the deposit.

The balance of the funds are required no later than October 10 2008, and can be paid in installments or in advance.

Please be prepared to bring with you some good used clothes, toys, school supplies, etc to take to the orphans and the poor.

NOTE: Payment can be made using Visa or Mastercard, but an administration charge of 4% will be charged by us for all credit card transactions (and if you are not paying with a Canadian credit card your bank may also charge you a processing fee).

Be A Hero reserves the right to deny any application.
 
WHAT IS COVERED: The cost of the trip includes accommodation in the trip location, three meals a day, and all transportation within the country.

WHAT IS NOT COVERED: You will be responsible for your travel costs, personal spending money, the cost of snacks and additional meals, and gratuities where appropriate (waiters, bus drivers, housekeeping, etc.), insurance, passport and visa fees, country exit tax, and immunizations where required.

FLIGHTS
You will need to book your own flights. The price of flights will be determined by where you fly from and when you book your tickets. We recommend booking flights as soon as possible to get the best deals. Please note that you will have to book your flights before you are able to obtain your visa.

VISA
You will need to obtain a tourist visa.  Full details of how to go about this can be found at the embassy web site.

It will take several working days for your visa application to be processed (excluding mailing times), so please apply for it in good time.

You will need to have booked your flights before you can apply for a visa.

Given the fact that visas cannot be purchased too far in advance, and as this is the high season for travelling please ensure you take out cancellation insurance with your flights for the event that your visa is not issued in time.

Please check the web site to confirm whether you must arrange a visa.  However, if you are returning to a country other than that of your own citizenship you will need a copy of your Permanent Residence Card or that countries’ visa.

Download the visa application from the embassy nearest to you – one form per applicant. You must get it signed and send in documentation.  Ensure you have all the relevant paperwork, photographs etc before you send in your visa application to avoid delay.

If you are under 18 there are additional requirements for travel – full details can be found on the web.  In summary – your parents must both sign the application for visa, and also give specific permission to travel, either in the presence of the Consular Officer, or it must be notarized. You will need to complete the form of authorization for minors to travel (even if your parents are accompanying you on the trip). Note: we will not accept applicants under the age of 18 unless they are travelling with an adult.

IMMUNIZATIONS
We advise you to talk to your own doctor about vaccinations that are recommended for your country.  The country of travel may ask visitors from certain countries to provide proof of vaccination against Yellow Fever.  (See the Consulate in your own country for information on this).

MANDATORY INSURANCE
You must have medical/health insurance in place that will cover costs in case of illness or emergencies whilst you are on the trip. This must cover the cost of air transport home in case of an accident/emergency. You must be sure that all installments are fully paid during your time with us.

Important Note: Be A Hero / Revival NOW! can not and does not guarantee your safety, health, or comfort. We are serving in a foreign country, in unpredictable conditions. You will be joining us in trusting God as provider and protector. You will not have the full attention of any missionaries, staff or translators. Please plan on being as self-reliant as possible. We see again and again that God does amazing work in each visitor. Be open to what God may do with you here! He is good.
